Applicant must be a recipient of OFW remittances. To ensure quick approval, prepare the following requirements:

• OFW’s employment contract
• Allotment slip (for sea-based OFWs)
• OFW’s passport 
• Government ID (Passport, National ID, Driver’s License, PRC License ID, Voter’s ID, Philippine Postal ID, TIN ID, OFW ID, New SSS ID, NBI Clearance)
• Payroll/remittance ATM card or passbook
• Sketch of residence with complete address
• 2 pcs. 2×2 pictures
• 3 months of recent bank statements of remittance account
